Overview
Kiaora is a direct-to-consumer telehealth platform providing personalized hormone therapy and metabolic health programs to women across the United States.
As the primary Full Stack Engineer, I worked across the patient-facing experience, backend services, subscription billing, data modelling, security controls, analytics, and production infrastructure.
Problem Statement
1. Complex Clinical Intake Process
Kiaora needed to collect clinically relevant information through detailed medical questionnaires. Lengthy forms and unnecessary questions could create friction and lead to patient drop-off during onboarding.
2. Complex Subscription and Prepaid Billing
The platform needed to support BHRT, GLP-1, and Thyroid programs, monthly subscriptions, prepaid multi-month plans, promotional discounts, split coupons, and account credits.
3. Referral Credits and Revenue Recognition
Referral rewards required eligibility checks to reduce self-referrals and duplicate rewards. Prepaid invoices also needed accurate revenue allocation across the correct accounting periods.
4. Sensitive Patient Data and Production Operations
The platform required controlled access to patient records, reliable deployments, production monitoring, security controls, and documented operational processes.
Solution Statement
1. Conditional Medical Intake and Autosave
Built reusable, multi-step medical questionnaires with program-specific conditional branching, debounced autosave, shared backend validation, and supporting patient onboarding flows.
2. Advanced Subscription Billing
Implemented Stripe subscription and prepaid billing, promotional-code resolution, split-coupon logic, customer balance handling, and post-checkout upsells without disrupting existing billing cycles.
3. Referral Management and Financial Accuracy
Developed referral credit workflows with fraud-prevention checks and corrected prepaid invoice revenue recognition using Stripe invoice period fields, including a backfill script for historical invoices.
4. Secure Infrastructure and Monitoring
Configured Supabase Row-Level Security policies, deployed NestJS APIs on AWS EC2 using PM2, optimized AWS Amplify builds, and implemented EventBridge and Lambda alerts to Slack.

What Sets This Work Apart
The project involved more than connecting a payment gateway. It required coordinating recurring and prepaid subscriptions, referral credits, promotional discounts, mid-cycle upsells, account balances, and revenue recognition within production billing workflows.
The patient intake experience was also designed around conditional branching and autosave, while analytics and UTM tracking improved visibility into acquisition and retention. Deployment pipelines, runbooks, monitoring, access controls, and team handoff processes supported long-term maintainability.
